Application for admission to the Residential Treatment
Center, the Therapeutic Group Homes or the Pathways Independent
Living Program is initiated by Ozanam's receipt of the following
referral material:
- Completed application form
- Social history
- Psychological testing results (WISC or WISC-R
and projectives)
- Educational reports (transcripts, PIAT)
- Audiometric screening results
- Physical examination results and immunization
report
- Reports of previous evaluation and/or treatment
programs
After this material is received, it is screened by
the Admissions Committee and, if the referral is thought to be appropriate,
an evaluation is scheduled. Interviews will include a psychiatric
examination and a social evaluation. Any additional testing necessary
will also be done during the visit.
If the adolescent is accepted for treatment at Ozanam,
an individual treatment plan will be written. If the application
for admission is rejected, the Admissions Committee will make recommendations
for referral to more appropriate sources.
